Hometown Heroes Banner Program- Nomination Form

The Mesa Chamber of Commerce is proud to support celebrating our hometown heroes through the Hometown Heroes Banner Program. Thank you for sharing the story of someone that has chosen to serve and to protect the freedoms that we all hold dear.

The Mesa Veterans Program, a committee of the Mesa Chamber of Commerce, will review all submissions once the application process closes. We will then contact the recipients for this year’s banners for pictures, additional details and information.                

We are currently accepting nominations for the 2024 display year. Please message us at info@mesachamber.org if you have any questions. We will accept applications through March 31, or until all spots are filled.

Banner recipients are expected to be announced the first week of April 2024.

Once contacted for required forms and photo I hereby grant the Mesa Chamber of Commerce permission to use the provided photo (which includes the likeness of myself, my relative, or my sponsored veteran or service member) in the Hometown Heroes Banner program, social media pages, official Mesa Chamber website, and other promotional material without payment or other considerations, which includes any photos shared with the Mesa Chamber regarding photos taken with the banners.

By completing this application, I agree that the Mesa Chamber is not responsible for replacing banners that are stolen, damaged, or destroyed due to age, vandalism, or any acts of nature including high winds. Any loss or damage to an installed banner will be handled at the discretion of the Mesa Chamber. Number of banner and location is subject to change.
